Social Prescribers
We have a team of 2 Social Prescribers who cover all three surgeries.
The team work flexibly offering a range of contact options including telephone consultations, home visits and drop in sessions. They support patients with non-medical needs including signposting to community services and programmes to improve their wellbeing.

Solent Mind Wellbeing Advisors
We have a dedicated team of three Mental Health Wellbeing Advisors and one Peer Support Worker. Together, they provide support for people who may be experiencing difficulties with their mental health or wellbeing. Services include:

Assessments and 1-to-1 Sessions
We offer up to 6 personalised assessments and ongoing support sessions to help you manage your mental health and wellbeing.

Our team can help you explore what matters most to you and guide you towards the right services, whether within our practice, the NHS, or the wider community.

Groups and Activities
We can also support you to access local groups, activities, and community resources that promote wellbeing and recovery.
Our aim is to work alongside you, offering practical and emotional support, so you can access the care and opportunities that best meet your needs.
Mental Health Nurse
Our registered mental health nurse works across Coastal PCN. Their role is to promote and support a person’s recovery, helping them live independent and fulfilling lives. They usually offer six fortnightly appointments and work closely with other teams including the Community Mental Health Team.

Please contact your surgery and discuss your needs with one of the care navigators who can refer you. Other members of your practice or PCN teams can also refer.
