About Us
Three practices have joined together to form Coastal (Fareham and Gosport) Primary Care Network [PCN]
Coastal Fareham and Gosport PCN has approximately 39,000 patients and serves a largely suburban population between Portsmouth and Southampton. We are equidistant between Queen Alexandra Hospital and Southampton General Hospital
The PCN was formed in 2019 and is focused on developing stronger links between the 3 individual practices plus the wider health and social care community. We believe this is the most sustainable way of ensuring our patient needs are met by the ‘right team, in the right place at the right time.’
What is a Primary Care Network?
Primary Care Networks (PCNs) are groups of GP practices working together to allow greater provision of proactive, personalised, co-ordinated and more integrated health and social care. Collaborative working and the development of new services which are specific to the PCN population aim to provide more targeted proactive(rather than reactive) care. Hampshire and the Isle of Wight Integrated Care Board (ICB) have 135 individual GP practices organised into 42 PCNs.
