Coastal PCN Pharmacy Team
The Team is made up of 3 Clinical Pharmacists and 2 Pharmacy Technicians. They work across all 3 surgeries within the PCN.
Clinical Pharmacists
Within primary care, clinical pharmacists work as part of a multi-disciplinary team (MDT) in a patient facing role to clinically assess and treat patients, using their expert knowledge of medicines to add value to and to improve patient care and patient outcomes. Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time. The Clinical Pharmacist role is pivotal to improving the quality of care and ensuring patient safety.
Pharmacy Technicians
Overall the main objective of the Pharmacy Technician role is to support the delivery of a safe and effective pharmacy service within the Primary Care network and to reduce the workload burden on GPs and Pharmacists.
Below are some of the areas Pharmacy Technicians can cover;
- Responding to patients’ questions both face to face and over the phone, referring problems or queries to the pharmacist / GP.
- Reviewing clinical correspondence (including discharges and clinical letters) and making the appropriate medication changes and arranging appropriate follow up.
- In line with the above, checking the accuracy of any changes made by Practice admin teams.
- Participating in medication audits, paying particular attention to patients who are prescribed more high-risk drugs and ensuring timely and effective monitoring is in place.
- Providing advice and information to GPs, staff, and patients to support the improvement of prescribing safety, quality, cost-effectiveness, and the appropriate use of medicines to get the best outcomes for patients.
- Assisting in the monitoring and management of prescribing budgets and the preparation of evidence-based resources and information that support cost-effective prescribing.

Alternatively if you have submitted an E consult to be triaged, it may be one of the Pharmacy team who call you to manage your query.
