The Home Visiting Service (HVS) is based at Stubbington Medical Practice and covers all 3 practices providing medical assessment for housebound registered patients and patents in nursing and residential homes.
The team is clinically supervised by Dr T Murray and is made up of Advanced Nurse and Paramedic Practitioners and Heather Tregale who is the PCN Frailty Lead.
The team visit patients in their own homes for assessment of acute medical issues. They also undertake chronic disease and medication reviews.
Many of the nursing and residential homes with the PCN have a named member of the HVS who visits on a weekly basis. Our HVS also co-ordinate COVID vaccinations for nursing and residential home residents and housebound patients.

If you are housebound and feel you need a home visit, please contact your surgery directly. Your details will be passed to the HVS for triage.
